Upload Records

Did you know you can upload medical records for expert review and evaluation? Please provide your contact information, the claimant’s name, claim number and any due dates we should know about.

Clicking “Browse Files” will prompt you to select a file, and you can select multiple files by holding down your “Shift” or “Select” key.

We can accept .PDF, .DOC, .PTX, .TXT, .JPG, .JPEG, .GIF and .PNG files. Please ensure that any imaging files are zipped prior to upload.

You will not receive a confirmation email when the upload is complete. Your documents have been successfully received when the form redirects and reads “Success! Your files have been submitted”.

If you are uploading more than 20 files please zip the files prior to upload.
